REGISTER AS A NEW PATIENT?
You will be asked whether your address is within our practice boundary. The practice area is up to five miles by road from Bacup town centre.
New patients are required to complete a registration form and to make an appointment with our practice nurse or health care assistant for a new patient interview. (You will be required to bring a sample of urine to this appointment and also a copy of your repeat prescription order form.) Children under five years of age will be given an appointment with the health visitor.
